BEETROOT JUICE AFTER 50: WHAT DIETARY NITRATE ACTUALLY DOES
A 2025 umbrella review of 20 meta-analyses found dietary nitrate improves time to exhaustion, muscular endurance, and peak power — and does nothing for time trials, VO2 max, or maximal strength. Here's the dose that worked, the blood pressure evidence, and the gap nobody mentions.
Beetroot juice has one of the larger evidence bases of any supplement in the endurance world. That evidence supports some outcomes clearly and others not at all, and which is which depends entirely on what you are asking it to do.
What Dietary Nitrate Actually Does
Beetroot is not doing anything magical as a vegetable. The active ingredient is inorganic nitrate. Once swallowed, some of that nitrate is reduced to nitrite by bacteria living on the tongue, and nitrite is then converted to nitric oxide in the body. Nitric oxide widens blood vessels and appears to make muscle contraction slightly more efficient — less oxygen consumed for the same amount of work.
That is the entire proposed mechanism. It explains both why the effects are real and why they are modest. Nothing about this pathway builds muscle, raises your ceiling, or changes your training age.
The Strongest Evidence We Have
In 2025, Sports Medicine published an umbrella review by Poon and colleagues — a review of reviews, pooling 20 systematic reviews with meta-analyses covering 180 unique primary studies and 2,672 participants. It is the most complete picture of this supplement currently available.
Nitrate produced statistically significant benefits on five outcomes:
- Time to exhaustion (standardized mean difference 0.33)
- Total distance covered (SMD 0.42)
- Muscular endurance (SMD 0.48)
- Peak power output (SMD 0.25)
- Time to reach peak power output (SMD −0.76)
Those are small-to-moderate effects. Real, repeatable, and worth something in a close race — not transformative.
Where It Did Nothing
This is the part that gets left off the label. In the same umbrella review, nitrate showed no significant improvement in:
- Time trials
- Graded exercise tests
- Maximal oxygen uptake (VO₂ max)
- Muscular strength
Read that list against how the product is usually marketed. The benefit clusters around open-ended efforts where you go until you cannot continue — not fixed-distance performances against the clock, and not maximal strength. If your event is a 5K PR attempt or your goal is a heavier squat, the evidence does not support beetroot juice as the lever.
Dose and Duration
Two details separated the trials that worked from the trials that didn't:
Minimum effective dose: 6 mmol of nitrate per day — roughly 372 mg. That is a concentrated beetroot shot, not a glass of juice from the supermarket shelf, and certainly not a beet in a salad.
Chronic beats acute. Supplementation lasting more than three days produced greater improvements than a single dose before an event. The common practice of taking a shot on race morning and nothing beforehand is the weaker version of the protocol.
The review also noted that nitrate salts were not as effective as beetroot juice or a genuinely high-nitrate diet — a hint that whole-food sources may carry something the isolated compound doesn't.
The Blood Pressure Question — Honestly
Nitrate's second selling point is cardiovascular. Here the picture is genuinely mixed, and the honest answer depends on who you are.
A 2024 meta-analysis in Nutrition, Metabolism and Cardiovascular Diseases pooled 11 trials and 349 patients with diagnosed hypertension and found beetroot juice reduced clinic systolic blood pressure by 5.31 mmHg (95% CI −7.46 to −3.16) versus placebo, with the effect holding up to 90 days. That is a meaningful number in a hypertensive population.
But the same analysis found the evidence did not support a sustained reduction across 24-hour ambulatory measurement — the clinic reading moved more convincingly than the all-day reading did.
And in older adults specifically, a 2024 randomized trial published in Food & Function gave beetroot juice for four weeks and reported that while nitrate metabolism clearly changed, vascular function and blood pressure did not improve.
The reasonable read: if you have diagnosed hypertension, there is a signal worth discussing with your physician. If your blood pressure is already well controlled, do not expect beetroot juice to move it.
The Gap Nobody Mentions: Almost None of This Was Tested on Us
The umbrella review's authors flagged it directly — there is a lack of studies focusing on youth and older populations. The nitrate evidence base was overwhelmingly built on young, healthy, trained subjects. Applying it to a 58-year-old masters athlete is extrapolation, not evidence.
Two further limitations from the same review are worth carrying with you:
- Sex bias. Female participants made up only 7–37% of subjects across the reviewed studies, and sex-specific responses have not been adequately explored.
- Methodological quality. Roughly 60% of the included reviews rated "critically low" on AMSTAR-2, the standard quality appraisal tool.
- Product variability. Commercial beetroot juice products show considerable variation in measured nitrate content — both between brands and within the same brand. The 6 mmol on the label is not a guarantee.
What direct older-adult data exists is small and early. A 2017 randomized, double-blind crossover trial in PLoS One gave 12 adults averaging 68.8 years a single 100g beetroot gel containing about 12 mmol nitrate. Muscle oxygen resaturation after exercise was faster (1.43 vs 1.02 %/s), blood volume during recovery was higher, and the drop in handgrip strength 30 minutes post-exercise was smaller than with placebo. Twelve people is a pilot, not a proof — but the direction is toward recovery, which is exactly where masters athletes have the most to gain.
The Protocol
- Use it for the right event. Open-ended, sustained-effort work — long climbs, muscular-endurance sets, tempo efforts you hold until you break. Not a strength cycle, not a VO₂ max chase, not a fixed-distance PR attempt.
- Dose at 6 mmol or more per day. Check the label for nitrate content in mmol or mg. If it isn't stated, the product isn't serious.
- Load for at least three days before it matters, rather than relying on a single race-morning shot.
- Test it in training first. Beetroot juice is a heavy, sweet, strongly flavored liquid and it will turn your urine and stool red. Neither is harmful; both are unsettling on race morning if unexpected.
- Talk to your physician first if you take nitrate-based heart medication (nitroglycerin, isosorbide) or blood pressure medication — this pathway overlaps with those drugs directly.
- Set the expectation correctly. This is a small edge on top of trained fitness. It is not a substitute for sleep, protein, or consistent training, and no supplement in this category ever has been.
